WP5 Culture of Safety

Assessing the Safety Climate/Culture

Objectives

To identify the effect of interventions, to improve handover at discharge, and the mechanisms or processes responsible for the implementation results of these interventions

To explore and assess cultural factors that hinder or facilitate effective handover at discharge or initiatives to improve handover (implementation of best-practices).

 

Description of work and role of participants

Over and over it has been shown that specific well-planned strategies are required to implement best-practices in daily practice. Most theories on implementation of innovations in health care emphasize that an analysis of the barriers to change practice is a prerequisite to develop an effective implementation strategy: effective implementation cannot take place without an analysis of the knowledge, skills and attitudes of the individual care provider and the setting and the target group in which the implementation is to take place.

Besides reviewing the effectiveness of interventions that aim to improve handover at discharge, and what makes interventions to be effective, this work package will analyze in-depth the cultural factors that hinder or facilitate the implementation of optimal handover practice. Culture reflects a shared set of attitudes, values, believes and norms of stakeholders in the handover process (e.g. interpersonal relationships, habits and structures).

 

Activities

The work package comprises the following activities:

Systematic review: We will conduct a systematic literature review to identify effective handover interventions and the mechanisms or processes that are responsible for the implementation results of these interventions.

Qualitative study: We will perform a systematic content analysis of focus group and interview data to explore cultural barriers and facilitators to effective handover and the implementation of best-practices.

Developing and validating a culture assessment model in the hospital – primary care interface: Using literature, focus group and interview data, we will construct and validate a culture assessment model to assess barriers or facilitators to effective handover.

 

Deliverables

D6 Report on cultural factors that hinder or facilitate effective handover or handover improvements (implementation of best-practices) at hospital discharge.

D10 Report on the effectiveness of interventions, to improve handover at discharge, and the mechanisms or processes responsible for the implementation results of these interventions.
